On the MENUBAR, hover your mouse on [Membership], a drop-down menu will appear. Click on [Membership Application].

On that webpage, Download the application, fill it out on your computer or by hand.
Enclose the application & dues ($10 per yr) and mail to:

On that webpage, Download the application, fill it out on your computer or by hand. Checkmark Renewal and/or Info Change. Enclose the application & dues and mail to:

As an alternative you can now pay your dues the F&A direct transfer.

On the MENUBAR, hover your mouse on [Membership], a drop-down menu will appear. Click on [Request Access to the website]. On that webpage, Fill-out the 5 questions, message and solve the simple math problem and click on Sign Me Up !

We usually are able to process your request within 2 hrs. (up to 24hrs)

Click on [Change my Password].

On the MENUBAR, hover your mouse on [Membership], a drop-down menu will appear.
Click on [Forgot my Password].

Login to the website.
On the menubar, go to [Member Access] on the drop-down menu list you'll find [Simple Member Lookup] click on it.

If you need to change your info - use the [Contact Us] on the Menubar.
This is a simple & easy way to confirm your membership information.

On the MENUBAR, click on [Contact Us] to update your info by email

Click on [Roster Purchase].

On that webpage, Fill-in the entries, print and mail the form along with $10 to:

Login to the website.
On the menubar, go to [Member Access] on the drop-down list you'll find [Simple Member Lookup].
If you need to change your info - use the [Contact Us] on the Menubar.
This is a simple and easy way to lookup a member.

You're not Logged in !
After you login the Members Access menu will become active.

Login to the website.
On the MENUBAR, hover your mouse on [Member Access], a drop-down menu will appear.
Hover over on [Resources], another menu opens, then click on [Sedgwick & W/C help].

One option members can select is “Lifetime” Membership.
Dues are a one time $300
You will never be billed for dues again.

The card is very specific on what you will need to do to get your account back on track. You can use the [Contact Us] link if necessary.

Which ever you preferred; handwritten, from a computer and printed. Sometimes hand written letters are difficult to read. Do the best you can.
All letters received are entered by hand into each issue of Broken Streams.

If there is something in your letter you do not want in Broken Streams, please let us know. For example, your contact information, address, phone, etc. Some would like it to be posted in Broken Streams, others not. Remember, Broken Streams goes to over 1500 members and ALL LACOFD sites. Let us know in your letter what you prefer. Whatever is written in your letter will go into Broken Streams unless you say different.

If you would like, you can Email a photograph, and if there is space, we will put in Broken Streams along with your story. The limit on size of a photo attached to an email is 10MB.
You can also mail a printed photo with your story.

On the MENUBAR hover over [Member Access] and a drop-down menu will appear, select [Calendar]

On the Calendar page, find your event by using the search box OR by clicking on the event from a displayed Calendar.

Once you're on the Calendar page you want, Scroll down the page to
'Leave A Reply' - pretty self explanatory from there.
(Yes, you can upload a photo also !)

Login to the website.
On the MENUBAR, hover your mouse on [Member Access], a drop-down menu will appear.
Click on [Resources], then click on [Retired Chaplains].

The Best place to start is call or email a Chaplain. They can get the process started. This also provides a means for the Department to come out with an official notification to active and retired members.

However, some family members do not want the “official” notification. In that case, contact one of the LACOFD Retiree’s Association board members to let us know please. Use the [Contact Us] link

We do recommend going through the Chaplain’s office to make it official. I know your family member would probably want his/her old friends to know about his passing. But it’s a family decision.

Send them here, to our website: LACoFDRetirees.org , or cut out our application on page 9 of Broken Streams, or take a picture of the application on page 9 and text it to them. Everything they need to know is on the application. There an online application under [Membership]
Note: The Department doesn’t notify us about new retirees. Some sort of privacy issue. So, your help would be much appreciated and Thank you!
